We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.</span></p></div><p><span style="font-family: verdana, geneva, sans-serif;"><span style="font-size: 16px;"><strong><em>While this role is primarily remote, candidate must be living in the tri-state area (NY, NJ, CT, and/or PA). Quarterly travel to Tampa, FL. 8:00am-5:00pm Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm.</em></strong></span></span></p> <p><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><strong>What You'll Do</strong></span></p> <p><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">As a Family Resource Specialist, you will provide direct support to patients and families in addressing social needs, working&nbsp;collaboratively with community partners and care team members to close gaps&nbsp;related to non-medical drivers of health. In this role, you will assist in building and&nbsp;strengthening community relationships, identifying and maintaining a robust</span><br><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">network of resources, and offering non-clinical support, guidance, and&nbsp;education to families navigating the unique challenges of caring for children&nbsp;with complex medical and behavioral health needs.
