In this role, you will:</span></p> <ul> <li style="font-size: 12pt;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Work as part of an interdisciplinary, collaborative team to ensure we are meeting patient needs</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Outreach to patients discharged from emergency rooms to ensure continuity of care and assist with identifying and meeting needs related to social determinants of health</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Deliver resources to families within your geographic area</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">
